What is SPORT Center?
SPORT Center is home to Whitefish Physical, Occupational, & Speech Therapy and Flathead Performance Training. Together, these businesses offer the Flathead Valley a comprehensive and unique approach to healthcare. Our staff at SPORT Center forms a complete therapy team with Physical, Occupational, Speech and Massage therapy as well as Athletic Training. Where is SPORT Center located?
SPORT Center is located in the North Valley Medical Village on the south-side of the new North Valley Hospital in Whitefish. Our physical address is 2006 Hospital Way in Whitefish, MT, 59937. Entrances to the Medical Village can be found on both Highway 93 and Highway 40 just before the intersection of these two roads. Click here for a printable map.
Can I fill out my paperwork prior to coming to your office?
All necessary paperwork can be viewed and downloaded in our Forms section. Please fill out the included forms and bring them to your first appointment along with your insurance cards.

At SPORT Center, you will receive one-on-one patient care with your Therapist or Athletic Trainer. Our appointments often last for an hour, giving your Therapist or Athletic Trainer ample time to truly evaluate and treat your needs. With a Physical, Occupational, Speech, and Massage Therapist as well as an Athletic Trainer on staff, we focus on fixing your mind and body as a whole, rather than just the parts. We aim to provide long-term solutions rather than offering just short-term relief. At SPORT Center, you will find a passionate and caring staff that treats each individual as a person rather than just a number. Remember, you have a right to choose your Therapist. If you feel that the SPORT Center is right for you, ask your physician for a referral to our facility.
Do I need to see my physician prior to coming to SPORT Center?
In the State of Montana you do not need a prescription from your doctor to see a Physical Therapist. However, to reimburse for services, your insurance carrier may require that you see a physician first. Currently Medicare requires a physician’s prescription before starting any treatments with a Physical Therapist. For both Occupational & Speech Therapy, you will need to see your physician prior to coming to our office. What should I do if I am un-insured?

What happens at my first appointment and what should I expect?
At your first visit we will perform a detailed evaluation to determine how we can improve your quality of life. Following our evaluation, we will discuss with you all possible treatment plans. From there, you and your Therapist/Athletic Trainer will create a plan of care that works best for you. We strongly encourage our clients to be active in their recovery and welcome your input throughout the evaluation and treatment process. We want you to feel welcome at SPORT Center, and rest assured that you will find a warm and friendly staff that truly wants to help you achieve your health and wellness goals.
What do I need to bring to my first appointment?
Please bring a copy of your insurance card, any paperwork you have filled out on-line, and a copy of your physician's referral if you have one. Please wear loose-fitting clothing and shorts if possible.
